What is included?

The vaccination sheet includes pet name, vaccine, date given, next date provided by your vet and clinic. The medication sheet includes medication, prescribed dose and unit, scheduled/actual time, status and notes. Each PDF page has 10 blank rows; each Excel sheet has 20.

These files do not send reminders or automatically sync with an AryNote account. Information you enter after downloading stays in your file; this page does not collect your completed records.

How to use the tracker

  1. Choose a format

    Download PDF for printing or Excel for editing on your computer.

  2. Identify the pet

    Write the pet name or a distinct identifier on each row; keep different animals’ records separate.

  3. Copy the document details

    Copy vaccine dates and prescribed doses from veterinary documents. The template does not calculate dates or doses.

  4. Separate planned and actual events

    Record scheduled and actual medication times separately. A blank row does not mean a dose was given.

  5. Store and maintain your log

    Keep the file somewhere safe and check its contents before sharing. Retain original booklets and prescriptions.

Common questions

Is this a vaccination schedule or treatment plan?

No. This is a blank record log. It does not decide which vaccines or medications to give or when; obtain those instructions from your veterinarian.

What if I need reminders?

Create records and reminders separately in AryNote. Excel rows are not automatically imported into the app.
